33 yo neck lift and revision failed - seeking guidance (Photo)

9mo ago had a deep plane NL. 32, not overweight, always had double chin even as a child w little body fat. Underwent revision w same PS, incisions behind ears only this time. He used stronger sutures. At 3 days post op looked great both times. Almost immediately after it "drops.” PS offered a 3rd revision w even stronger sutures, ears & chin incisions. States area feels tight so not skin but muscle dropping/sutures not holding. same issue. Could it be a gland issue?
