Help! My left breast feels hard, raised scar, painful. My right breast feels soft, less raised scar, no pain. Uncomfy sleeping on left. Is it capsular contracture? Too much side boob = capsulopexy? I'm 31 yo petite Asian female, 5'2", 115 lbs, non-smoker, healthy, no kids. BA done 1.5 years ago. Preop: 34B / Postop: 34D. I want to downsize my 350/325cc high profile to <250cc to reduce at least one cup size. What is cost range for capsulopexy? Fixing capsular contracture? Breast lift? Thank you
Answer: Capsulorrhaphy, implant exchange are good choices for you Hello and thank you for your question. I agree with your concerns that your breasts look too big for your frame. With smaller implants and capsulorrhaphies, your cleavage can be brought closer together and also give you breasts that are perky but more proportional. Your scar can also easily be revised. Whether or not you have capsular contracture would be determined with an in-person examination, but if you do have capsular contracture, your options are a neosubpectoral pocket or a capsulectomy. Don't let the details bog you down at this point; just find a board certified plastic surgeon with expertise in revision breast techniques who you feel comfortable with and your situation can be improved. I hope this helps and best of luck to you!Sincerely,Dr. Sean Kelishadi

Answer: Capsule revision, breast implant exchange Thanks for the description and photos. Of course, as others have said, only an in-person examination can permit a plastic surgeon to fully counsel you. It appears that although you have a good overall aesthetic result in terms of breast shape with mild asymmetry, you are concerned about the overall size and the painful scar. The easy part first: the painful thickened scar is a hypertrophic scar that may respond to steroid injections, scar revision, scar revision + steroids, and/or scar revision + radiation. If you are contemplating having the implants exchanged, the scar can be excised at that time and reclosed. A recurrent thick scar could still occur but you do have an opportunity for revision of the scar at the time of implant exchange. The breast augmentation may need downsizing, capsule repair and or capsule repositioning. Sometimes this costs more than the original augmentation. Examining the skin, breast volume and symmetry in person can tell us a lot. We may be able to use surgical simulation software to give you an idea of what kind of result may be possible with subtraction of the current volume and addition of a range of smaller volumes.Happy to help!

Answer: Quote Pricing varies around the country depending on the area you are in. I would say where you live is one of the more expensive areas. Your scar likely needs to be excised and closed again but it appears to be a hypertrophic or keloid scar and cutting it out can make it worse. Sometimes a little Kenalog steroid injection will help it. Seek a board certified plastic surgeon for a revision quote and treatment plan.

Answer: Downsizing Breast Implants From your photos, it appears that the implants, although relatively balanced, are too big for your aesthetic goals and frame. You can downsize these to a smaller and more streamline appearance, but this must be determined after an in-person consultation. The capsular contracture possibilty and hypertrophic scar also must be evaluated in person. This can be corrected, but how you heal may impact the outcome. Thicker scars are more common in Asian patients. You should see a board certified plastic surgeon in your area to get a formal opinion through an in-person consultation. Best of luck, Vincent Marin, MD San Diego Plastic Surgeon
