I currently have 700cc implants and I'm looking to upsize to 1100-1200 range. I have saline under the muscle and I have a pretty wide frame. Im 140lbs, 5'5". I weightlift regularly and keep active as well. What should I expect from this surgery and does this seem reasonable?

Dear samihaglam,breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery.Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size.While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. The implant selection should be based on a combination of your desired size, soft tissue dynamics, breast base diameter and skin elasticity.  I recommend an in-office examination as well as a detailed discussion with a surgeon who you are comfortable with. If you are maintaining the same breast pocket, then that surgery should be very straightforward.  How much volume you can add will be limited by the pocket yourself and your frame.  Some patients can go from 700cc to 1200cc in one setting without difficulty.  Others can only go 200-300cc more at a time before the tissues are too tight to handle more.  I would discuss this with your surgeon prior so that you have a good idea of what limitations there may be.  I hope this helps.

Make sure you meet only with board-certified plastic surgeons in consultation. This is a larger volume which would have to be an over-filled saline implant. It is not a matter of exchanging implants but will involve alteration of the breast tissueDr Edwards

Thank you for your question. It is generally my feeling that bigger implants yield bigger problems. Particularly saline implants that are overfilled. It may interest you to know that the FDA is currently investigating larger silicone gel implants (up to 1445 cc). They currently are only available for breast reconstruction through the study, but at some point in the future, they may be available for cosmetic augmentation patients as well.  I would still discourage you from going that large, but if you sincerely wish to do so, consider waiting until the larger silicone implants are available. Good luck.

Thank you for the question. You will find that online consultants will not be able to provide you with specific advice in regards to selection of specific breast implant size/profile. 
Ultimately, careful selection of plastic surgeon and careful communication of your goals (in my practice I prefer the use of goal pictures, direct examination/communication in front of a full-length mirror, and computer imaging) will be critical. 
Generally speaking, the best online advice I can give to ladies who are considering revisionary breast augmentation surgery (regarding breast implant size/profile selection) is:
1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. Ask to see lots of examples of his/her work.
2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. 
In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural or fake” or "D or DD cup” etc means different things to different people and therefore prove unhelpful.
Also, as you know, cup size varies depending on who makes the bra; therefore, discussing desired cup size may also be inaccurate. 
3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Again, in my practice, this decision is usually made during surgery ( after the use of temporary intraoperative sizers). Viewing the patient's chest wall in the upright and supine positions, with temporary sizes in place, help select the best breast implant size/profile for the specific patient.

Going with such large implants is usually not good long term. Please do not choose implants that are to heavy for your frame and tissue. Implants of 1100-1200 cc's will not fit your 5'5", 140 lbs frame well. Otherwise, your own tissue will thin and the implants will just sag. Big breasts sitting on your lap are not pretty. Also, the larger saline implants are, the more chance you will have rippling.

There are several ways to get the fake look-either with overfilled saline implants, custom XL silicone implants or XL tissue custom expanders.It depends really on what cc size to want to get.
You can think of a saline implant as a balloon. The manufacturer recommends an overfill of about 20cc but they have been shown to be able to be filled much more than that. They do start loosing their shape and can significantly distort the appearance of your breasts. 
 I do overfill espcially since the FDA has restricted saline and silicone implants to a maximum saline shell size of 800cc and maximum silicone size of 800cc. I do not know why that number was picked arbitrarily. 
So, I do do breast reconstructions greater than that with XL tissue expanders which do pride a greater diameter and projection and upper pole fullness and allow the size to be adjusted either up or down. Also, the shell of the saline implants are very thin and can dreflate more easily.
