The DAO (Depressor Anguli Oris) pulls the corners of the mouth downward and slightly outward when you frown. Botox injections in this area can weaken the muscle enough to allow the corner of the lip to rise a little. The injections are medium depth, about 2-5 mm deep. Before injection, I ask patients to turn their lip down so that the muscle can be identified. I inject just to the outside of the marionette line to avoid hitting the DLI (Depressor Labii Inferioris) muscle which overlaps with it in the inner area. We used fine insulin needles without any issues. If patients are nervous, we apply a topical numbing cream for a few minutes, but most patients do not require this.
