For top surgery in the Greater Seattle area, Dr. Day is all I could have asked for. He's very compassionate and knowledgeable. He also answers questions thoroughly, both before and after the procedure; I especially liked when he drew diagrams to explain the possible incisions during my consult. Getting in contact with his office is a pretty smooth process, and his assistant, Gentry, is super friendly as well. I'm writing this review almost two months after the procedure because I'm nothing if not a chronic procrastinator, but it does mean I can definitely say that the results are flawless. There are a number of different types of arm, axillary, and breast lift techniques, some of which are performed in combination with each other. It would take a full exam to say specifically which would benefit you most, but an excisional procedure of your excess skin seems to be needed. Everyone's body tends to store fat and exhibit excess tissue in various areas at different tendencies, and some of these area can be stubborn despite (or, paradoxically, sometimes because of) diet and exercise. So when those benefits have been maximized, then maybe it is time for removal of excess sagging tissue.

You pose the question essentially as tummy tuck OR liposuction, but I would encourage you to discuss the possibility of tummy tuck AND liposuction with your surgeon. This is otherwise known as a lipoabdominoplasty. This technique needs to be performed taking care not to too aggressively remove fat, as the abdominal flap of tissue that is created needs to have its blood flow maintained in order to heal well. However, a well-executed lipoabdominoplasty can combine the benefit of deflation of fat along with removal of excess tissue.
There are typically sizing charts for most compression garments, such as Isavella, which for this scenario would have measurement of the chest circumference under the breast. You can actually order these readily online if you have tape measurements, or some offices, such as ours, will obtain these for their patients.
First off, this is a common question and a good and fair one...but it is also a little tricky to give a reliable answer to for a number of reasons. Cup size is a good (and bad) "language" to discuss postoperative size expectation after breast reduction, because the cup size of each bra differs with the bra manufacturer, let alone determining an accurate expectation from your surgery. That being said, the size you're targeting is the common goal of those with large breasts causing back pain and other symptoms. There are also tools and guidelines, such as body mass index, body surface area, and what is called the "Schnurr scale," which is used by insurance companies to differentiate breast reductions from breast lifts (which are not covered by insurance). So that is probably wear those numbers came from. Ask your doctor and communicate your wishes, but the size range you mentioned is reasonable and likely achievable.
