So I don’t mind my nose but I feel so insecure when I laugh or talk that it makes this shape and I look like a villain. Is there anyway to help it without surgery like Botox? I noticed I’m the only person whose nose does that and everyone’s nose stays still but it’s made me so insecure please help!
Answer: How do I stop my nose from look like a V-shape or it dropping so much when I talk or laugh? Hello and thank you for the question and sharing your photos. I believe you are a good candidate for rhinoplasty. You have a bulbous tip and alar flaring, which gets worse when you smile. You will benefit from nasal tip refinement and alar base reduction. My best advice is that you discuss your concerns and aesthetic goals with a board-certified plastic surgeon with extensive experience in performing rhinoplasty. I would ask to see before and after pictures of their real patients. It's important that you trust your surgeon's skills and ability to achieve the results you are looking for. In particular, pay attention to your surgeon's experience with rhinoplasty. Best, Pejman Aflaki, M.D. Johns Hopkins-trained double board-certified plastic surgeon

Answer: Rhinoplasty Hello and thank you for your question. Based on your photographs, you may benefit from a tip refining rhinoplasty. Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. You may also benefit from a cephalic rotation of your tip to turn up your tip slightly and placement of a columellar strut graft to help prevent drooping of your tip when smiling. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. You should be able to view numerous pictures of actual patients with noses similar to yours performed by your surgeon. If you cannot find enough real before and after pictures, then consider continuing your search for a surgeon. Best wishes and good luck. Richard G. Reish, M.D. FACS Harvard-trained plastic surgeon

Answer: Botox for nose Hi Andrea, thanks for your good question about this. I agree that, since this a 'dynamic' issue - i.e muscles in your face make the nose change shape when you make certain facial expressions - that turning these muscles off with botox injections to the sides of the nose would be a great thing to try. due to coronavirus restrictions, there is no cosmetic tratment or face to face consultation going on at the moment but most PSs are doing phone / facetime consults. best wishes Oliver Harley

Answer: Tip definition, slight uprotation and caudal septal resection is needed Hello, According to your complains and the pictures you have provided, you are suffering from the term''hanging columella'' when you smile, and it is possible to solve this problem with a tiplasty, redefining the tip, resection of the caudally tense septum and a very slight uprotation. As well you may benefit from fillers in order to uprotate the tip and prevent from hanging but it is not a permanent solution and as it only prevents hanging columella it wont be sufficient for tip definition. So the best option would be a good tip plasty preserving your nazal valve.Good luck
