My nose doesn't have a lot of projection or definition, and it makes my face look really heavy. I attached edited pictures of my nose and a model's nose that I love. Are my expectations too high for what's medically possible?
Answer: Should I use filler or surgery to get my dream nose that has more projection and definition? Hello and thank you for the question and sharing your photos. I believe you are a good candidate for a surgical rhinoplasty, focusing on nasal tip refinement. There are a variety of techniques that can be utilized, including trimming the nasal tip cartilage, suturing techniques, cartilage grafting or combination of those. My best advice is that you discuss your concerns and aesthetic goals with a board-certified plastic surgeon with extensive experience in performing rhinoplasty. I would ask to see before and after pictures of their real patients. It's important that you trust your surgeon's skills and ability to achieve the results you are looking for. In particular, pay attention to your surgeon's experience with rhinoplasty. Best, Pejman Aflaki, M.D. Johns Hopkins-trained double board-certified plastic surgeon

Answer: Should I Use Fillers Or Surgery To Get My Dream Nose? Thank you for this intriguing question. Obviously, a surgical rhinoplasty would give you the best, most long lasting results. On the other hand, a liquid rhinoplasty can improve your lip/nose relationship, give you the appearance of a relatively sharp tip, and lengthen your columella. A liquid rhinoplasty can also give you an idea of what your nose would look like after a rhinoplasty. You can wear it for a short time or a long time, and then wipe it away by using hyaluronidase. If you were my patient, I would put Voluma in your columella in the area just at the bottom of your nose known as the peripyriform area and onto the tip of your nose to give it the appearance of a tip graft with a supertip depression. This would improve your lip/nose relationship as well as camouflaging your overly projected upper lip. If you know just the nose you're looking for, then choose a Board Certified plastic surgeon with years of experience doing surgical rhinoplasties. If you have any ambiguities concerning the final appearance of your nose, try a little filler. it may surprise you how good fillers look and how much information they give you.

Answer: Rhinoplasty Surgery vs Fillers Aloha and thanks for your question. It appears you would be a good candidate for Rhinoplasty to narrow and refine the size/shape of your nose. Fillers would probably not help change your nose in any significant or long-lasting way. I would recommend you see an experienced Surgeon Certified by The American Board Of Plastic Surgery[ABPS] and a member of The American Society of Plastic Surgeons (ASPS) for a consult. Mahalo, Dr D

You can have a more projection and definition to your nose , but NOT a thin skinned nose like the model whose skin is very thin. Answer: Rhinoplasty Hello and thank you for your question. Based on your photographs, you may benefit from a tip refining rhinoplasty which can be performed by trimming, suturing, and reshaping the cartilage in your lower nose. You may also benefit from a cephalic rotation of your tip to turn up your tip slightly and a dorsal hump reduction. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. You should be able to view numerous pictures of actual patients with noses similar to yours performed by your surgeon. If you cannot find enough real before and after pictures, then consider continuing your search for a surgeon. Best wishes and good luck. Richard G. Reish, M.D. FACS Harvard-trained plastic surgeon
