Which procedure can help me correct a crooked columella? (photos)

Hello! I’ve had a crooked nose tip for a long time, over a decade since my early 20s at least, but it seems to be looking worse. I don’t know if I broke it at some point and it didn’t recover correctly or what, but it’s just now bothering me aesthetically. What are my options for getting the nose I should have had?

Hello! Thank you for your question. As far as I can see from your photo you have strong dorsal hump and very droopy tip. Thankyou for providing pictures as a reference. In my professional opinion you may benefit from closed atraumatic rhinoplasty which can address your concern in the least invasive way without cutting the tip of the nose. Additionally, your nose can be straightened and the tip can be reduced and defined and if you have any functional issues, they can also be addressed at the same time. It is possible to correct it. The majority of our patients with rhinoplasty request surgery because they have curved noses.

There appears to be asymmetry of the lower lateral cartilages of your nasal tip. Freeing these from their attachments to each other and the septum and securing them in a symmetric fashion may be all that is necessary. If your septum is deviated caudally (towards the end) that should be repaired at the same time.

A rhinoplasty to just reshape your tip cartilages would be ideal.  I believe the issue is your medial crura which are prominent and slightly asymmetric.  With sutures this can be corrected.
