I have high cheekbones, but my skin underneath the cheekbones is bulging out. I think that a buccal fat removal will help the case, but this wouldn't address my narrow mouth. When I pull my skin underneath the highest point on my cheekbones towards my ears, this tightens the area underneath my cheeks and gives a nicer look as I already have nice cheeksbones, in addition to streching my mouth towards the sides, and hence widens my mouth and provides am overall balanced look - 20 year old

Answer: Widen mouth and tighten area under cheeks w/ a mini face lift? Possible for a 20 year old? Hi, I have performed many SMAS facelifts for over 30 years and have performed many minimally, invasive SMAS facelifts. Non smiling photos of your face from the front and side would help in the evaluation. If "jowls" are present in the lower face as described below (very unlikely in a 20 years old but I have seen it in 27 year olds) a SMAS facelift would be considered. There are no procedures that would aesthetically widen the mouth. The lips look best when they are full and properly proportioned with the upper no more than 3/4's the size of the lower lip. A balanced chin, on profile, will also highlight the lips. Following my beauty principles, women look the most feminine, youthful and attractive with heart shaped faces. Heart shaped faces have cheeks that are full and round in the front. Cheek augmentation with a dermal filler or using cheek implants for a permanent enhancement will create full, round cheeks that will feminize the entire face. Conversely, men look chiseled and handsome with angularity in the cheeks, chin and mandibular angles. If you have "jowls” these are sagging facial tissues and an indication for some form of a SMAS facelift. The underlying SMAS layer, of the face, must be dissected, lifted, trimmed and re-sutured (not merely folded or suspended with threads or sutures that will not last). The excess skin is then removed and the facelift incisions closed.
