There is something going on with the corners of my mouth. A dermatologist told me that I have perleche. She gave me a steroid and then told me that she should fill the corners with a filler in order to help my situation. There is definitely something going on with the corners of my mouth. Could someone explain what is happening aesthetically? Also please comment if fillers or something else would benefit me aesthetically and if there is a possibility that these fillers could cause blindness. Thank you.
Answer: Angular Cheiliitis - Fillers around corners It appears that you have angular cheilitis in the corners of the mouth. This can be caused by certain type of infections ie candida, bacteria which can be simply treated with a topical antibiotic. In other situations, it can be caused by different vitamin deficiencies such as Vitamin B2, Vitamin B3, zinc, or iron. If this is reoccurring it is probably due to vitamin deficiencies. I would recommend a good quality multi B supplement (2 per day), zinc 50 mg/day, and iron glycinate until at least it clears. From an aesthetic viewpoint, the lost of volume is due to a natural lost of volume in the corners of the mouth which causes the drooping of the corners of the mouth. The angular cheilitis, if onging and recurrent, can cause more destruction of tissue over time and hasten the emergence of this problem. The solution to this is add a HA filler (Juvederm, Restylane, Beletero) to these corners to provide structural support. Beletero looks the best but has the shortest duration of the 3. Juvederm may last the longest of the 3. In addition, Botox may be needed to also help to lift the corners.

Answer: Corners of mouth great question, thank you for submitting. from your photograph, it appears as though in addition to the perleche, you have a bit of fat wasting at the corners of your mouth. By using a small amount of hyaluronic acid filler, it will reduce or resolve the shadows at the corners of your mouth. Blindness is an extremely unlikely scenerio from filler in that location but using a hyaluronic acid filler allows the doctor to use an enzyme dissolver if there is any issue. It is a very well tolerated area that makes a tremendous difference. I would recommend seeing a board certified dermatologist or board certified facial plastic surgeon who does a tremendous amount of this type of procedure and who has a good artistic eye for this procedure.
