I have a deviated septum but no breathing issues. I feel that my nose has become more crooked over the last year or so an has developed a bump on the bridge. The surgeon who I just consulted with for rhinoplasty doesn't want to touch my septum and just wants to fix the nose cosmetically since I can breathe. To me, that seems like building a house on a foundation that isn't level. Do you think the septum needs to be fixed during the rhinoplasty so that it doesn't continue to get more deviated?

Answer: Rhinoplasty and septoplasty Hello and thank you for your question. It depends on how bad the septal deviation is and if your nose crooked also. Even if you are not having breathing problems, your nose may be crooked and it would be a good idea to have a septoplasty at the same time. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. Best wishes and good luck. Richard G. Reish, M.D. Harvard-trained plastic surgeon

Answer: Septoplasty with rhinoplasty The nasal septum is a critical structural component of the nose. Even if you are not symptomatic from your deviated septum your surgeon should consider straightening it at the same time as rhinoplasty. A septoplasty is often done to ensure you dont develop a breathing problem from the rhinoplasty procedure and to avoid any adverse rhinoplasty outcome such as a crooked nose. In certain situations your nose can become more crooked post nasal hump removal if the nasal septum is not adequately straightened.
