Fold/crease in nipple 3.5 weeks post Gynecomastia surgery, what can help? (Photos)

Question

I seen my surgeon yesterday, and he told me to began tissue massage as the nipple retracted because it was very puffy prior to operation. He recommended “vitamin e cream, or cocoa butter” . How can I get the nipple creases to smooth out and connect with the rest of the nipple? Would tissue massage be better later then now as it may cause swelling? *i also put a lot of neosporin on them so the creases are much worse when it’s dry,

Your early results look pretty good. I instruct my patients to massage but you should follow your own surgeon's instructions. Hang in there and be patient. It takes a least 3 months to see the final results.

Hello! Thank you for your question. Based on your post and on your photos, it seems that you are still swollen from surgery. Allow the swelling to resolve, and let your body heal. (In most patients, this takes at least 3-4 months.) Once that happens, you and your surgeon can discuss your concerns (if any). I hope this answers your question. Best of luck!

Hello and thank you for your question and photo's .   At 3.5 weeks you're still healing and swollen .  Give it 3-4 months to see the final result, and that crease may not be there when all is healed.

Thanks for writing,Your crease over time will improve greatly. You are very early in the healing phase. For total scar production it take six weeks, so you are barely into the maximal production of scar. For scar to fully mature it takes 18 months. I tell all my patients, the scar will look worse before it looks better, because of the above. What makes scar better? Time, pressure, massage, Silicone gel or sheeting, Vitamin E (some people question this, including me), Steroids (you do not need this). Application of Neosporin is a waste of time and it is only exposing your skin to antibacterial ointment that you don’t need. I tell people to lightly massage as often as they think about it. Scar works 24 hours per day, and we massage 4 to 5 times per day for 5 10 minutes. Guess who is wining the game? The scar, so light massage help. Massage stimulates an enzyme called collagenase which help to convert early scar (type III collagen) to mature scar (type 1 collagen). This does not happen over night, this is a long term commitment.This will improve, be patient. Follow your Board Certified Plastic Surgeons post-op instructions carefully and completely.
