Canthopexy after surgery gone wrong? (Photo)

Question

My doctor says that the Line beside my eye Can be fixed by a Canthopexy surgery. I Had removed some eyebags 1 year ago and Left me with this long scar/Line. And round eyes with Sclera to show.

You do seem to have bowing of your lower lids on the sides and a small hooding of your upper lids over the scar. As you lift the brow with your finger, the hooding is improving. A canthopexy could improve your scléral show but to improve the lateral hooding, you would need a lateral brow lift or upper eyelid skin tightening. An examination could confirm these findings.

A canthopexy will do nothing for this scar.  As you can see in one picture when you hold up your brow, you have a heavy lateral brow which makes this scar appear deeper.  By elevating the brow with some Botox, or a surgery, the scar will be less noticeable.  You can also either do a scar revision to just excise and reclose the scar, or do some  laser on the scar.

Hi,Thank you for including photos. It is very unlikely that a canthopexy will help with the line/scar. Typically, a canthopexy is performed when the lower eyelid has lost its tightness. You will need a careful examination of the eyes to determine if you would benefit from a canthopexy. I recommend bringing a photograph of yourself prior to the surgery to determine your eye shape prior to surgery - this can guide the surgeon and assist with planning. Good luck,Mehdi Sina, MD

I perform a canthopexy in most patients especially those with lax lower lid tone. This will support the lower lid and minimize the risk of scleral show.  It will not improve the scar issue.Your lifting or pulling up or laterally suggest a lift of the brow may improve the holding laterally.

So canthopexy is one option to lift your lower eyelid so it doesnt slope downhill. If this is from an overcorrection of lower lid skin however, a canthopexy alone is unlikley to improve things for you. Hope that helps. Adam Goodwin

This is a situation where an in person evaluation is crucial.  It appears you have a negative vector (eye sticks out far past orbital rim) this making your lower eyelid support compromised.  It is unusual for a canthopexy to be enough.  The whole lower eyelid needs to moved upward, only than can the corner (canthus) be a sharper angle.  I am least worried about the scar And most about the eyelid position itself in these cases.  I can not tell enough from photos, these cases need to be seen and palpated  in person.
