Hi. I hate my nose and am determined to have rhinoplasty. I have not yet chosen my surgeon, but I feel I need to know whether my expectations are realistic before I go for consultations. My nose is wide and the tip is turned down. I want a natural lookingnose, a bit narrower and tip not down. I have attached photos of my nose . Can this be done and if yes would it be considered a relatively easy or a complicated surgery bearing in my mind every surgery has risks. Thanks.

Answer: Are your expectations realistic? Are your expectations realistic? You appear to be a very good candidate for rhinoplasty. Your simulation seems basically realistic and close to what could be achieved. If the end result falls short it will be that your simulation may have a millimeter more tip projection then your final result may have.

Answer: Realistic expectations Just based on the photos, you appear to have realistic expectations and you could be a great candidate for a rhinoplasty surgery, though a physical exam would be necessary to be certain. As a board certified plastic surgeon with decades of experience in rhinoplasty surgery, I see patients with nasal configurations like yours daily. They key to rhinoplasty surgery is to look like no one touched you and for the result to appear so natural that you look like you were born with your result! In your case, you straightened the bridge of your nose, eliminating the bump. This could be accomplished by filing the bones (rather than breaking them). The lower part of your nose has been addressed as well, and the confluent straightness to the tip could likely be achieved by trimming/manipulating your lower-lateral cartilages. You also increased your projection of the tip, which a columellar strut would address. Overall, while “real life” surgery can’t exactly replicate what photoshopping an image creates, I believe that in the hands of the right surgeon with decades of experience, a straighter profile with a more feminine tip could be achieved. I recommend consulting in person with a board certified plastic surgeon to discuss your goals.

Answer: Nasal hump The results are realistic but a surgeon can never replicate exactly a photoshoped computer image. As long as you are aware of these limitations you can get a rhinoplasty with a good result. Regards Dr. J
