I've had issues with my nose since I was a kid, but it's really begun to cause major issues lately. I have trouble breathing out of my left nostril and my nose is overall crooked, droopy & uneven. I've always been self-conscious about my nose and I want to make a change to have a nose that fits my face better and is more symmetrical but doesn't drastically alter my appearance. Would I be a good candidate for rhinoplasty/septoplasty and would my case be considered complex?

It is challenging when a patient wishes to obtain someone else's nose, or wants such a drastic change that it would alter their entire appearance. This situation often leads to disappointment for both patient and surgeon, as both the changes themselves and the associated expectations are often not realistic. When a patient prefers a more "natural" outcome and wishes to preserve their identity rather than change it, I find that they are frequently in the right mindset to take on this important decision. The fact that you have breathing issues is also important, because you would want to make sure that both issues (aesthetic and breathing) are addressed by the surgical plan you come up with with your rhinoplasty surgeon. I think too often these issues are separated into distinct problems when in fact they are actually the same thing - you only have one nose after all. In this regard, I think of these cases as "complex", as the functional issues and aesthetic issues need to be addressed simultaneously. One thing the photos you showed can't demonstrate is the degree of septal deviation or other internal nasal issues that may be present. Based solely on your photos, it appears that you would be a good candidate for a rhinoplasty, though a history and physical exam are necessary to be sure. If you are having difficulty breathing through one nostril, you very likely have a deviated septum which could be addressed at the same time as the rhinoplasty via a septoplasty. A septoplasty is also useful because it provides material for me to manipulate to improve your tip projection. As a board certified plastic surgeon, I consult with potential rhinoplasty patients on a daily basis, and rhinoplasty surgery is one of my favorite operations to perform. Male rhinoplasty differs from female rhinoplasty since the bones are more formidable, and the outcome should favor a result that is handsome and masculine. Your nose is quite unique in that the tip is very blunted, which is likely one of the elements that have made you self-conscious as you’ve described. This is due to a very short columella that “hooks” your nose down and produces a convexity on the dorsum that is asymmetric. The bony elements of your nose can be filed down, and the cartilage trimmed and reshaped to address the dorsal hump and droopy, crooked tip. Projection is an important key in your case, and I agree with you that a rhinoplasty should look very natural, as the nose should complement your features.
