Should I do a lift? (Photo)

Your nipples are located below your breast fold and are pointing downward. A lift would be recommended since larger implants would not provide a sufficient amount of lift on their own.

I think most people would be extremely disappointed getting an augmentation and no lift, according to your photographs. The expectation should be that your nipples won't change position much. They may even become lower. You look like an excellent candidate for breast augmentation with lift, which would also be able to decrease the width of your areola, if you desired.

based on your photos, you look like a good candidate for breast lift. My preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity.

Choosing large, over-the-muscle implants like 630cc may offer some filling and minor lifting, but they likely won't significantly change the position of your nipples or correct pronounced sagging. If your nipples currently point downwards, this may persist post-implantation. For a more noticeable lift and nipple repositioning, a surgical breast lift may be necessary. Discuss your aesthetic goals with your surgeon to determine if a lift is needed alongside implants to achieve the look you desire.

No matter how big, implants will not give you a lifted or perky breasts; your nipples and tissue will hang off of those very large implants. You need a true breast lift, go to someone who can give you an excellent result, with or without implants.

Only cause your breasts to sag more. It will not give you a lift. I think you can get away without a lift as long as you understand that your breasts will be a little in the low side.

The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. I would not recommend implants to you as you have enough volume, you just need a lift.

Kelly, I appreciate your desire to avoid scars on the breasts however, it appears from your photographs that breast lift with implants would, by far be your best option.
630 cc implants are quite large and while placing them over the muscle will give you some lift, it will also provide a very large and unnatural look for your breasts. Placement of smaller implants under the muscle, combined with a lift will allow your surgeon to shape and lift your breasts in the most attractive way, and provide a much more natural look. You may consider seeking a second opinion, with a board-certified plastic surgeon in our area to explore this option. Best of luck.

No, you will just have larger and still sagging breasts because of having such a large and heavy implant. You would do much better with a lift and smaller implants placed behind the muscle for support and upper breast fullness. What minimal lift you will get in the beginning will not last. Go back and talk to your surgeon.

If the nipple sits below the infra memory, fold, a lift is often needed. A large implant will lift the breast slightly.
630 mL is a substantial implant. I generally recommend patients have multiple in person consultations before scheduling surgery. The decision to have a lift or not is something you should discuss with your provider. Some plastic surgeons prefer to do these staged procedures for safety reasons. You can always have a lift down the road if you don’t feel your nipple position is acceptable after the procedure.
These are decisions you should make with your provider.
