Why do I have hard and apparent lumps near my eyelids 8 months after blepharoplasty? (Photos)

Question

I got a blepharoplasty in October 2021 and the incision has healed well. Unfortunately, I now have these big lumps near my eyes. I also (stupidly) got brow fillers done at the end of December. I think the fillers must have transferred- I have some puffiness in my left eyelid and now my eyelids look uneven. Questions: 1. Are these lumps from the fillers or some fluid stuck in the eyelid area? 2. Is there a way I can get rid of the lumps? 3. My surgeon told me to use contractubex, would you agree?

Hello @coolysar thank you for your question. It is important to clarify with your surgeon if the substance placed intradermally in the eyebrow area is hyaluronic acid. If that is the case your surgeon may recommend putting hyaluronidase in the area to dilute the substance and prevent further migration. Massaging the area may also help to to dilute the substance. From the history and from the photos provided, it is difficult to say exactly what is going on here.  An in-person examination would be very advisable. However, in general, if it the lumps felt are very firm, they might be hypertrophic scar tissue from the surgery or the delayed appearance of lumps from the filler treatment. Hypertrophic scars can be reduced with intralesional injection of an anti-inflammatory agent and HA filler lumps with hyaluronidase+/-antiinflammatory agents, as needed depending upon the response. Softer, "pillowy" lumps, such as you describe in the left eyelid may be due to obstruction to lymphatic drainage. This is generally more difficult to treat, but I have had success with the use of hyaluronidase along with a varying combinations and concentrations of anti-inflammatory agents. Make sure that you consult with an experienced, board certified cosmetic dermatolgist. Hope this helps.

My answer about this issue is to wait and to massage over there. Filler can be moved there; the most devastating problem is that if the filler goes to upper eyelid, it can cause ptosis. In that condition, there is a need a surgery for removal of the filler.
