Will augmenting the chin through genioplasty 5mm vertically and 6mm horizontal advancement fine for a female? What is the maximum vertically lengthening you perform for female with horizontal advancement?

It is hard to give you a better answer without any photos to go on. It depends how recessed and short your chin is. Many surgeons these days are able to simulate outcomes for you using computer programs, and they go based on what looks the best, not necessarily focusing on the numbers. If your chin is very recessed, you could potentially be a candidate for jaw surgery as well. You may think about how your bite fits together or if you have any problems sleeping (snoring, apnea). If you are strictly looking for cosmetic procedures, then a genioplasty is typically more beneficial than an implant and can hopefully be simulated for you before surgery. The amount of vertical lengthening depends on the amount of deficiency. I use a CT scan to measure the vertical height from the bottom of the chin to the tip of the lower incisor. The normal length for a woman is 40-42 mm. So for example if the vertical height is 36 mm then a vertical lengthening of 4-6 mm can be accomplished. The advantages of the sliding genioplasty are that you can move both vertically and horizontally with the same operation. A vertical lengthening of 5 mm is very common in our practice.
