There are multiple ways to correct the view; 1- Remove the bridge and leave the top alone. 2- Leave the bridge alone and elvate the top of the bridge 3- Gradually increase the top of the bridge combined with gradually decreasing the bridge. all of the options can be done with the rhinoplasty surgery. Hello! Thank you for your question.I have evaluated your photos. I highly recommend you a rhinoplasty surgery. It is the best way to get rid of your dorsal bump. Your wish picture is realistic. As you mentioned, filler is temporary and it may lead to many complications (such as blindness as you said, many people are not aware of that).

If something -- filler or a graft -- is placed above the bump, it just makes the nose look much larger and more prominent, and I'll bet that's not what you're looking for. The bump is removed in a rhinoplasty, but importantly, the nose is evaluated to make sure it still looks right and natural with the tip position, width of the bridge without the bump, etc.
How *much* to lower the bump, or make other changes? We figure that out with morphs, well before surgery. I'd love for you to see some excellent professionally-designed morphs of what could possibly be done with your nose. Morphs could also help you identify better just what's bothering you, and help you set a goal for the rhinoplasty that's accurate for your tastes. Profile and three-quarter views would be particularly important in morphing your nose. (Side note: in my opinion, morphs should really be done by the surgeon, or he should direct an assistant as she makes the morphs. Morphs should be made with a constant eye to what actually *can* be done in surgery, for that particular nose, and the surgeon has that information and judgment best.)

Finally, remember that rhinoplasty is an exquisitely difficult operation to get right, and you should only have surgery if you are able to make yourself very confident in your surgeon's skills. You are a good candidate for dorsal hump reduction rhinoplasty. I would not recommend augmenting it to ‘hide’ the hump. If the hump is the only thing for which you seek improvement, a closed technique approach would be feasible.
