Hi I have put on some weight in the past couple of years and recently have begun to notice slight jowls that are appearing on my face. I can’t tell if this is because of the weight gain or my skin is starting to sag and I would love to know some remedy to this, is it surgery? I’ve also developed fine lines on my forehead and I don’t know if Botox is the answer? I’ve never had any of these procedures on my face before and I’m concerned that it should not look on natural with weight loss or gain.
Answer: Multiple answers As we age we have the above mentioned concerns: jowling and increased laxity of the skin. This accentuates the cervicomental angle and blunts it. You can have hollowing under your eyes. Your brows may drop. A comprehensive discussion addressing the above concerns is key. Botox may be sufficient for your forehead with a laser peel. The neck in my opinion is almost always best treated by a surgeon who can address the superficial (skin, subcutaneous fat) as well as the platysma and deep neck anatomy (submandibular glands, interdigastric fat and digastric muscles). Each patient has different needs and goals. The jowls are best treated with a facelift. There are many modalities that will temporize your concerns. Seek out a good surgeon who can not rush and go into further detail to your specific needs. Unfortunately, there are no good shortcuts. Best of luck on your search! For your forehead: botulinum (such as botox) is a great place to start. It will help but you should stick with it. For the early jowls and your neck (which is coming down as well) you could get a facelift, or, you could do something called "embrace". This is an office procedure that takes about an hour and will tighten the skin and neck.
