Greetings Physicians, I am a 53 yr female, non smoker, in overall well health. Kindly advise treatment options to improve the appearance of my neck and jowls due to age, gravity, loose and saggy skin. Stay well.

Thank you for your question. A lower rhytidectomy, or neck lift, would likely be an ideal procedure to address your skin laxity concerns. This option is less invasive than a full facelift and requires smaller incisions to directly target the skin on the neck and under the chin. A neck lift may improve your skin’s elasticity and reduce the “turkey neck” symptoms you describe to reveal the natural contours of your jawline. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon who can assess your needs on a more individual basis.

Thank you for sharing  your questions and photos. You have moderate age-related sagging of the soft tissues and skin of the lower face and neck. While many nonsurgical options are available, they would yield limited / insufficient results based on your degree of facial aging. Treatment would generally require neck and lower facelift. These procedure address three issues: loose neck skin, jowling/loss of jawline definition, and sagging in the cheeks / nasolabial folds. Results are highly predictable, especially if choosing a board certified plastic surgeon or facial plastic surgeon that performs a large number of necklift and facelift procedures. As a side note you may consider discussing subnasal lip lifting during your consultation as you may be a candidate for this as well. The length of your upper lip may be disrupting the proportions of your lower face. Correcting this at the same time (with a subnasal lip lift) would contribute synergistically to the result of lower face and necklift. I hope this answers your questions. Thank you again for sharing. Based on these limited photos, it seems that lower face/neck lift would be a procedure of choice for you. This will include correction of the anterior neck, jawline/jowls and lower face. In person consultation with a board certified plastic surgeon, expert in face lifts, is recommended. Good luck.

I would say that the most improvement to your neck could be obtained with a facelift.  This procedure addresses the lower face and neck and would tighten the area along your jawline as well as your neck.  There are some non-surgical options that could lead to improvement that would be less than that achieved with surgery.  One of these non-surgical options would be Face/NeckTite and Morpheus8.  These are treatments that use radio frequency to deliver heat that causes tightening.  Face/NeckTite would tighten the tissue under the skin, while Morpheus8 would tighten the skin itself.

A full set of facial and neck photographs from all angles are required to make the best determination about how to proceed. From the very limited photographs presented, it appears that a lower face and neck lift would be helpful. In our practice, this procedure involves tightening loose facial and neck skin, tightening loose facial and neck muscles, lifting the jowls, and removing fat deposits in the neck located above and below the platysma muscle in addition to a platysma-plasty to significantly improve the jawline. You also appeared to have a significant pre-Jowl sulcus which makes your jowls look bigger than they really are. Another option for this issue would be placement of the pre-Jowl sulcus chin implant in that area
