I’m still just in my late 30s so perhaps too young and not yet a candidate for a facelift. But I’m bothered by this fold or hollow or crease I have under my left eye area, around the apple of my cheek. I have done under eye fillers in the past, years ago though. I also did voluma in my cheekbones twice. But I don’t know why I have this weird ridge/crease thing and my doctor doesn’t offer me any solutions. Is this possibly correctable?
Answer: Swelling This is a classic problem with fillers in the under-eye area. This is edema related to swelling of the filler and/or obstruction of the lymphatics. I would recommend dissolving the filler. I would recommend seeing a board certified or board eligible plastic surgeon with aesthetic surgery training/experience who can perform a full exam and provide safe recommendations.

Answer: How to treat a crease under the eye: Know your options Hello, and thank you for your question. Based on your photo, it is difficult to determine if the crease is natural or a result of your previous filler treatment. To determine its cause and receive proper treatment, I highly recommend only seeking consultations and treatments from a reputable board-certified facial plastic surgeon, particularly one with extensive experience with injectables. Such experts will know what product to use, how much product to use, and where to precisely inject them while having an eye for symmetry and facial harmony. During your in-person consultation, your surgeon can assess your skin and recommend the best approach to safely achieving your desired results.

Answer: Management of full area below eye Hello! If you have had filler treatments in that area in the past, my best guess is this is focal filler that is visible under the skin. The "tear trough" area of the face has very thin skin and is quite unforgiving with filler treatments. I would recommend you see a board certified plastic surgeon to examine the area. If it seems to be filler, then it may be treated with an enzyme that can break down the product and smooth the area, almost immediately. Best of luck to you!

Answer: Lower eyelid crease Without a physical exam, it is difficult to fully respond but regardless, this area would not be helped with a facelift. You might need to consider hyaluronidase to dissolve some of the filler that possibly migrated to create this ridge. You also might be suffering from edema due to clogged lymphatics, a very unfortunate side-effect of undereye fillers. If the hyaluronidase does not work, and it is determined that the irregular area is not caused by edema, perhaps you need filler placed below the ridge to camouflage it. Never, but never use any filler under the eyes that is not an HA (hyaluronic acid) filler. I recommend consulting in person with a board certified plastic surgeon to discuss your concerns and to see what can be done. If possible, bring photos of yourself before the filler to diagnose your current issue.

Answer: Eye crease I am not certain but based on the picture you may have some residual filler above the crease, causing this issue. I would suggest you see a board certified physician experienced with injectables to explore this possibility and the possibility of dissolving that product if indeed it is the cause. Good luck!
