A BBL can sometimes soften the waist-to-hip transition and add some buttock fullness, but the result is limited by how much fat can be safely harvested and how much survives. If you want a very round, high-volume, traditionally feminine buttock, fat transfer alone may not be enough; some patients consider staged fat grafting or buttock implants, but implants have their own risks and maintenance issues. The other important point is that surgery changes your shape all the time. If you like looking masculine day to day, a large BBL or aggressive waist/hip shaping may not match that goal in clothing. A more subtle plan could involve selective liposuction of the waist and flanks with conservative fat transfer to the hips and buttocks, or no surgery if the tradeoff feels too permanent. An in-person consultation with a board-certified plastic surgeon experienced in body contouring and gender-affirming goals would help you map out what is realistic for your anatomy and what degree of change fits both sides of how you want to present.
