But the best surgical technique will depend on your skin quality, nasal structure, and aesthetic goals, as well as a face-to-face evaluation with a surgeon.

I recommend scheduling an in-person consultation with a reputable board-certified facial plastic surgeon, particularly one with a strong rhinoplasty portfolio and experience with Asian rhinoplasty. Such experts will have knowledge of nasal anatomy, as well as an aesthetic eye for facial symmetry and balance. There are several options for you that can be determined after examination. In my practice, I utilize piezosurgery. Piezosurgery is a more gentle technique that allows for greater precision. Less bruising and swelling is common with Piezosurgery. In a typical rhinoplasty, a hammer and chisel technique is performed leading to more inadvertent fractures.

During your consultation, your photos can be computer-morphed to show how the changes to your nose will affect your appearance. DCF (Diced cartilage fascia) provides permanent results without the risk of warping or resorption of en bloc rib cartilage, and with a much lower rate of migration, infection, or extrusion than synthetic grafts. With a rhinoplasty surgeon experienced in this technique, diced cartilage wrapped in fascia is a consistent, reliable, long-lasting technique for creating dorsal augmentation with excellent results. It is difficult to tell from those photos. Typically, an Asian nose needs to be built up along the bridge because it can be somewhat flat. Your photos also show your nasal tip to be wide, so rhinoplasty can make the tip thinner and bring in the nostrils some as well. Be sure to see a rhinoplasty expert for consultation.

The best technique for your nose will depend on what the goals of your nose are.

If you are looking for a higher bridge and tip, then you are likely to require an open rhinoplasty with dorsal augmentation. If you are concerned about the width of your alas, you may require an alar base reduction.

Different surgeons have different methods and approaches to rhinoplasty. First determine what bothers you about your nose and what you would like changed and seek multiple consultations for different options and go with the one that will help you meet your goals.

There are many techniques used in Asian rhinoplasty. From your pictures I think you need your nose narrowed and the nostrils narrowed as well. I would perform a closed rhinoplasty without cartilage grafts. I suspect others would use grafts and there’s nothing wrong with that either. Just make sure you go to an experienced surgeon

A full set of facial photographs from all angles that are in focus are required to make a determination about how best to proceed. A closed rhinoplasty approach can accomplish making the required changes to your nose, such as refinement of the nasal tip, and narrowing the bridge line and shaving down a dorsal hump. An alar plasty is performed to narrow wide nostrils. It's also important to have realistic expectations when there's thick skin present in the nasal tip. If you're having breathing difficulties, a thorough internal examination of the nose is required to see if you have a deviated septum or some other issue creating breathing problems. Both functional and cosmetic nasal surgery can be performed simultaneously when needed. Digital computer imaging would be helpful at the time of a consultation to understand what Can and cannot be accomplished.
