There are several options for you that can be determined after examination. In my practice, I utilize piezosurgery. Piezosurgery is a more gentle technique that allows for greater precision. There are many techniques used in Asian rhinoplasty. From your pictures I think you need your nose narrowed and the nostrils narrowed as well. I would perform a closed rhinoplasty without cartilage grafts. I suspect others would use grafts and there’s nothing wrong with that either. Just make sure you go to an experienced surgeon
A full set of facial photographs from all angles that are in focus are required to make a determination about how best to proceed. A closed rhinoplasty approach can accomplish making the required changes to your nose, such as refinement of the nasal tip, and narrowing the bridge line and shaving down a dorsal hump. An alar plasty is performed to narrow wide nostrils. It's also important to have realistic expectations when there's thick skin present in the nasal tip. If you're having breathing difficulties, a thorough internal examination of the nose is required to see if you have a deviated septum or some other issue creating breathing problems. Both functional and cosmetic nasal surgery can be performed simultaneously when needed. Digital computer imaging would be helpful at the time of a consultation to understand what Can and cannot be accomplished.
