Which procedure would be best for targeting my neck area? 37 y/o (photos)

I am very self conscious of what I have going on in my neck area. I am not that* old at age 37 and not overweight 136 lbs at 5'6 size 4 pants. I am desperate to fix this so that I can feel confident especially in photos I can't stand to even look at them anymore :( please suggest honest outcome expectation if I pursue surgery? I am very healthy but do have Hashimoto's thyroiditis for low thyroid. I don't know if that can cause the neck area to look like this but overall the rest of skin is ok

I would suggest either a moderately invasive technique, such as FaceTite or other skin tightening devices, or a neck lift. The neck lift will be more reliable but will certainly entail more in terms of cost and down time. Given you age, your skin is likely to contract well with a moderately invasive procedure, however only an in person physical exam will be able to tell whether or not your skin is of appropriate quality for this.

Thank you for your question and for sharing your photographs. Hashimoto’s thyroiditis does not create problems with the texture of the neck skin. There is a moderate amount of fullness along the jawline and, more significantly, under the chin. Part of the volume under the chin is hereditary and not related to aging, but it becomes more noticeable as the texture of the skin changes.

The deep plane neck lift procedure can combine volume reduction under the platysma muscles with a more conservative approach to volume reduction under the skin. The fatty tissue situated under the skin is important because it allows for softer contours, especially in women who have more delicate skin in the chin and neck region.

Depending on the characteristics of the result you wish to achieve, combining facelift and neck lift surgery can be beneficial. The effect on the contour of the jawline and labiomandibular lines is more powerful when the neck lift is complemented by the facelift.

You have beautiful facial features and you have developed several issues: saggy skin that wrinkles in the middle line, saggy platisma muscle which created the thick  two “cords” on your neck, played the bulging volume under the chin area which most likely is the combination of digastric muscle thickness, subplatisma fat (under the muscle which never be reached with liposuction) plus I think you might also have some protruding submandibular glands which prevent the nice definition of the jawline so you are a perfect candidate for Deep plane Neck lift with trimming the glands, digastric muscle, trimming of the subplatisma fat and tightening of the platisma muscle plus The skin resection using an incision behind the ears.

There are a spectrum of options available for you for neck contouring, depending on tissue quality and if any prior procedures have been performed to the area. I think the most impactful procedure would be a necklift. The necklift would take care of the extra fat, loose muscles/bands and loose skin. There are technologies to treat loose skin or excess fat if you want to take a more conservative approach.

A surgical neck lift accomplishes conjuring the neck itself by removal of fat deposits, located both above and below the plasma muscle, liposuction, and tightening the plasma muscle in the front of the neck with a plasma plasty procedure. No skin removal is needed. If you have loosen and elastic skin in the neck, then lower face and neck lift would be required to lift the jowls, tighten loose facial and neck skin in addition to tightening loose facial and neck muscles
