After a revision to further refine tip (which didn't happen), I am left with a nose that is nothing like what I wanted or began with. I'd like to lengthen my nose and rotate it back down as well as further refine my tip . My nostrils now flare upward and would like them to be brought back down and smoothed. I never thought I would lose the aspects of my nose that I loved and would like to know if I can improve this. I know I can't have my old nose back. What can be done? Thank you.

Firstly you need a surgeons who specialises in rhinoplasty as the majority of their practice. In general this sort of surgeon will have significant experience with rib graft use and be able to explain the benefits and limitations
I recommend that you wait another 7-10 months before considering another operation. A revision takes longer to heal, you had extensive work with multiple grafts 5 months ago, and you have several problems. I'm sure you're frustrated but you must be patient so the next operation is the last.
The bone can be rasped under local anesthesia to reduce the residual hump. A poly beak is a very, very unlikely complication of this procedure. However, increased width of your bridge is possible if a significant amount of bone is removed without doing osteotomies to reposition the nasal bones.
