Lifting nose can easily be done in office as minor procedure under local anesthesia although oral sedation with pills is helpful to make the experience less stressful. In some cases the tip can be deprojected (tip rhinoplasty) with local anesthesia and oral sedation but that involves a formal tip rhinoplasty which is more involved. An in person consultation would be necessary. Most surgeons would prefer to do this in operating room under deeper levels of anesthesia.
If by "local," you mean injecting the nose to make it numb, *and some IV sedation*, then yes, we never use inhalation general anesthetics for rhinoplasty or revision rhinoplasty. If you mean being wide awake and having a five-minute procedure, then no: your operation would be hours of complicated surgery, and you wouldn't want to do that wide awake. It can't be done with filler injections, and it's a complicated, but do-able, procedure
Revision rhinoplasty to deproject the tip etc. under local anesthesia? No! This would not be smart since other parts of the nose need to be done to balance the nose. See a very experienced revision rhinoplasty surgeon to discuss specifically what will need to be done to achieve your goals.

Revision rhinoplasty is more challenging than primary, so general anesthesia is prefered for patient's comfort and safety. I do a lot of revision rhinoplasty procedures and I don't advise you to have this done under local. It's uncomfortable and you're less likely to get the results you want. Consider a consultation with a rhinoplasty specialist to discuss your goals and how to best achieve the nasal appearance that you want. Best wishes
NO, revision rhinoplasty and primary rhinoplasty procedures are performed under general anesthesia for patient safety and comfort. Rhinoplasty is a surgical procedure performed in a formal operating room, not the backend of a clinic. The hanging columella can also be reduced in addition to decreasing the projection of the nose. The photographs demonstrate a recessive chin profile. Consider placement of a chin implant to augment the chin forward for better facial balance and proportions, especially with respect to the overly projecting nose from the side profile.
My first response is NO, but I have done minor tip revisions under local anesthesia. It does depend on exactly how much change you want. I need to examine a patient like you and discuss the options. You ask if it can be done in the office; yes in the office operating room.
It seems that in taking down your dorsum, your nose was shortened a bit which led to a bit of upward tip rotation... your slightly negative profile is exaggerated by the fact that your tip is slightly over-projected.... although you 'could' augment your dorsum a bit, perhaps the best approach is...

Hi @pjannahx! This is an excellent question. Spreader grafts will not change the size and shape of the nostrils. Spreader grafts are used to improve breathing. If there is nostril collapse, an alar batten graft can be used. Best of luck, Dr. Nima!
