Question

I’m 36 year old trans woman, 5’7, 115lbs. I’ve had some facial feminization work done to my face already. I have been considering a facelift because I have these shadows in my face that I notice in pictures. For years I have been consistent with my skincare routine & sunscreen, retinal, vitamin c Ect. I have a very hard time gaining weight tho & lost around 10 pounds this last year due to my busy schedule & putting off meals.

Thank you for your query. You seem to have more of a volume loss than sagginess of your facial skin. Volume replacement with fat or fillers is a better option for you than a surgical face lift. I do a lot of facial feminization surgery. a deep plane facelift may be a good option but there are others. We may be able to adjust the fat and tighten the skin other ways.

Based on these photos it does not look like  a face lift would change much? In person consultation with a board certified plastic surgeon, an expert in facial rejuvenation, may reveal something that is not visible in these photos? Good luck.

From your concern I recommend Mini- facelift or Mid facelift procedure. This operation tightens excess skin and relies entirely on tension to shift the underlying facial soft tissue, the excess skin will be reduced to balance for lifting and tightening the cheek muscles. This is done under General Anesthesia with 2 weeks recovery time.

Facial aesthetics, especially in young people are primarily determined by facial skeletal structure. Period
To make an assessment we need a complete set of facial pictures, including profile pictures.
Soft tissue manipulation may be helpful with the use of treatments like fillers.
I do not see a good indication for facelift surgery.
