The tip can be narrowed as well as the alar base. The dorsum can be narrowed as well. Alar base reduction by itself is probably not going to satisfy you. Find a plastic surgeon with ELITE credentials who performs hundreds of rhinoplasties and rhinoplasty revisions each year. The tip and nostrils can be narrowed/ improved using your own tissues. Your bridge may need narrowing or height built up without implants - you can use your own tissue. The nose may have better proportions with the bridge treated at the same time as the tip and nostrils.

You have what is called "cephalically oriented" lower lateral cartilages. This means that the tip cartilages point more up than out. It creates a parentheses shaped shadow between the tip and the nostrils, causing the nostrils to appear more prominent. If the cartilages are reoriented and the nostrils reduced slightly, you will get the result you are wanting.
Alarplasty, surgery on the nostrils, can help narrow the nostrils. A tiny amount of skin is removed from the nostrils, and the small incisions are hidden within the contours of the nose. Less is more with alarplasty or nostril narrowing procedure, as small changes in the skin can make relative larger changes in appearance. Alarplasty may be performed in the office via local anestheisa or combined with other rhinoplasty procedures via general anesthesia. Improving definition of the tip is achieved with cartilage contouring and/or grafts. Only after a comprehensive evaluation can a rhinopalsty surgeon help determine appropriate options for you. An alarplasty will improve the width of the nostrils, but your tip and bridge should also be addressed during surgery to maintain an attractive balance. Projecting the tip of your nose and making it slightly taller will make it appear smaller and more refined from the frontal view.

Thank you for your question. I recommend an open rhinoplasty with augmentation of your nasal dorsum with a cartilage graft from your ears. I would also refine your nasal tip and narrow your ala or nostrils. This can all be done under local anesthesia with sedation. You will have great results, good luck!

Hi, Reduction of your nostrils are necessary but that will make the tip look even proportionately larger. Therefor the tip must also be improved and cartilage grafts are used to support and bend the skin to a more refined shape. This one picture is limited and I would need to see your base view and profile and feel the skin of your nose and the tip cartilages as well but you can have a nice nose without an implant if you see the right surgeon.

That area is specially sensitive after surgery, however it is doubtful that you have caused any real harm to the area. However, I would check in with your physician to make sure that you are healing properly.
