Lowering a hairline can reduce the size of the forehead. This can be accomplished through both hairline lowering or with transplants. For those patients that want hairline lowering, but the scalp is not flexible enough to lower it to the point where they want it, scalp expansion is also potentially an option.
Forehead reduction surgery or hair grafting transplant can both lower your hairline. It would be in your interest to have a consultation with doctor(s) who perform these types of surgeries.
Based on your photograph(no frontal view), you appear to have three issues.1. High hairline: hairline lowering surgery, when performed optimally should result in a scarless and natural hairline. However, your hair growth pattern and hair style may preclude you from getting an optimal result; the goal is to have hair follicles grow through the scar and thereby camouflaging it optimally. 2. Your upper frontal(forehead) bone is prominent; after a CT scan to ensure there is no underlying sinus cavity, it can be easily reduced in surgery.3. Finally the area between the top of your nose and prominence of your forehead is recessed and can be augmented (permanently) with fat grafting, which will also slow further aging(atrophy) by augmenting blood supply to the frontal bone. This would bring your profile(and frontal view) into balance, reducing the apparent size of your forehead.Hope this helps
I think you would benefit from hairline advancement and contouring of the forehead bone. This is all done through the same incision at the same time. The results are excellent if done by an expert. Your forehead is prominent for several reasons. The hairline is high, the bone is overgrown, and the junction of your nose to your forehead (radix) is low. To achieve the very best results, all of these things would need to be adjusted. Access to the bone of the forehead for contouring is through an incision in your hairline. Once the bone is sculpted, the hairline is brought forward when it is closed. For your nose, I can't tell if any other changes would be needed, but I would add cartilage to the bridge of the nose to create a smooth transition between your forehead and nose without a depression. Your large forehead can be reduced by lowering your hairline, a procedure we frequently do in African American ladies. I suggest you consider the Irregular Hairline Incision technique that we described many years ago. The hairline is lowered immediately with this technique and the patient maintains normal uniform hair density.
Forehead reduction surgery comes in two types, the first is a surgical procedure that gives you same day results, the second is a hair transplant that takes 6-7 months to see. With regard to hair loss following such surgeries, in women it is not common, but it does happen and is a known risk.
