I am getting Natrelle Smooth Cohesive gel 400 cc implants with a sub-glandular under the crease incision. I have asked for Moderate profile but I cant decide if Moderate plus is better for me? I have posted 3 pictures of what i look like now. The last is my ideal look. My ideal look had subglandular 350 cc high profile.

Based on your ideal look picture, I would recommend the moderate plus profile. Most women in my practice go with the moderate plus instead of the moderate.

Answer: Moderate plus or moderate will make little difference.
The chest wall appears to be fairly large so should be able to accommodate a 450 mL moderate profile implant. Moderate plus would be satisfactory as well.

Answer: Breast Implant Profile
The same size implant may look different in a different patient. I like to use three dimensional imaging to show patients a simulation of what the implant sizes and profiles look like. A picture is worth a thousand words. A higher profile implant of the same volume will have a smaller width. Typically, I prefer to use implants close to your natural breast width.

Answer: Best Breast Implant Size/ Profile for Me?

Despite good intentions, online consultants will not be able to provide you with meaningful specific advice. Ultimately, in person consultation with well experienced board-certified plastic surgeons will be necessary. Careful measurements, dimensional planning, and careful communication will be key.
In your case, based on your photographs and stated goals, I think you are starting at a great starting point for breast augmentation surgery and should have a very nice outcome. Keep in mind, as you consider your goals, that the most common regret after this procedure is “I wish I had gone bigger”. I would also suggest that you carefully consider the pros/cons associated with breast implants placed in the sub glandular versus sub muscular ( dual plane) positions.
Generally speaking, the best online advice I can give to ladies who are considering breast augmentation surgery ( regarding breast implant size/profile selection) is:
1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you're looking for. Ask to see lots of examples of his/her work.
2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals.
In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or “C or D cup” etc means different things to different people and therefore prove unhelpful.
Also, as you know, cup size varies depending on him who makes the bra; therefore, discussing desired cup size may also be inaccurate.
3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Again, in my practice, this decision is usually made during surgery.
