Question

I recently had lipo . I'm really satisfied with my results but absolutely hate my belly. It just never got flat after surgery it's not as saggy any more but is not flat flat like I want it! I was considering a tummy tuck and a breast reduction/life My breast also got really big after pregnancy and naturally started to sag. The size is just to much for me so I also want a reduction but not by a lot. My nipples still face forward they don't point down. Not sure if that effects the procedure

Yes it seems like you would be a very good candidate for a mommy makeover. You can get a tummy tuck and breast lift at the same time and this is part of what people talk about when they use the term  "mommy makeover".

Of course, these procedures can be done separately though the cost is usually more. It is very common for women to want a mommy makeover, especially after having had children. Pregnancy definitely is hard on the abdominal and breast areas and this is a comment complaint among women visiting my office.

I would recommend that you see a board-certified plastic surgeon in your area that is a member of both ASPS & ASAPS and performs a lot of mommy makeovers. Good luck with your future surgery!

Healthy people at their ideal weight (a BMI less than 30 to reduce the risks of complications during and after surgery and to enhance results) should have the best overall recovery from a MM, but some of this will depend upon the surgical technique and adherence to postoperative protocols. 
A TT with muscle tightening will create a flat abdomen and would also provide the benefit of placing your incision along the lower pubic region. If the waist was smaller at one time, the waist can likely be reduced to those same measurements or smaller provided the muscular and fascial integrity has remained the same. 
BR/BL procedures involve removing the extra skin and repositioning the breasts in proportion to overall body weight.

I would begin with your breasts using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. This technique will make you look taller and thinner. Liposuction does not change the intra-abdominal fat content or increase the mechanical strength of the abdominal wall. If you are interested in a flatter abdomen I recommend abdominoplasty with mesh reinforcement, if you are not planning future pregnancies. The mesh creates a more hourglass figure by compressing the abdominal wall resulting in early satiety and helps maintain a reduced weight.

I tell my patients that liposuction will not fix the skin or the rectus diastasis that can happen after being pregnant. Once those abdominal muscles are repaired with a tummy tuck or abdominoplasty, then the abdomen can look flat again plus removing the excess skin will make it even more smooth. A small breast reduction to elevate the nipples can usually be done at the same time. Keep in mind that if you do the tummy tuck alone to remove tissue and flatten the abdomen, it gives the illusion that the breasts look relatively bigger. Make sure to see a board certified plastic surgeon.

Thank you for sharing your question and photographs.  Based on your goals it seems you would be an excellent breast reduction/lift and tummy tuck candidate.  The breast lift/reduction can variably adjust your breast size while removing excess skin, allowing for a more youthful appearance.  The tummy tuck will allow for tightening and flattening of your stomach muscles.  To get the best results with the least risk I would work on continuing your weight loss journey.  Hope this helps.

Tummy tuck with flank contouring liposuction combined with breast lift/reduction appears to be an excellent option for you. The tummy tuck will maximally flatten your stomach and contour the region nicely. The combination breast lift/reduction will allow your plastic surgeon to both lift and attractively reshape your breasts to make them perkier and more attractive. Hello and thank you for your question and photo's.  If you have had pregnancies, a tummy tuck will help bring those abdominal muscles together , however, it also can be intra abdominal fat that diet and exercise will get rid of .

Thank you for your questions and your pictures. I understand that you are happy with your liposuction results but you thought your abdomen would be flat once it was fully healed. Unfortunately, liposuction only removes the fat on top of the muscles and doesn’t actually tighten the muscles. When assessing the abdominal muscles to see if they can be tightened surgically. I first have the patient lay down to assess if the abdomen gets flat when in that position. If the belly is still sticking out, it is likely that there is intra-abdominal fat , meaning fat around your organs in your belly, that is making the abdomen protrude. It is very difficult to flatten this type of abdomen, even with a tummy tuck. Initially the abdomen will look flat after surgery but the abdomen gradually stretches back out because the fat is still present inside the abdomen pushing out. If the abdomen flattens when laying down, I then have the patient lift their legs off the table, holding them a few inches off the table. This maneuver will demonstrate weakness in the abdominal wall like rectus diastasis and umbilical or ventral hernias. All of these can usually be fixed during the abdominoplasty, if the umbilical hernia is large it may not be adequately fixed and require further repair after the abdominoplasty. A thorough abdominal exam by a board certified plastic surgeon would be needed to fully assess if you are a good candidate for an abdominoplasty. Moving on to your question on your breasts. If the nipples are facing down, a Mastopexy or breast lift, would definitely be indicated. The choice of whether or not to remove breast tissue, and how much breast tissue should be removed, would be up to you. These decisions would be completed at your consultation. A tummy tuck can remove excess skin as well as repair abdominal wall separation, which is not done with liposuction.  A tummy tuck can be combined with a breast lift/reduction if you are generally healthy and a good candidate.  See a board certified plastic surgeon near you to learn more.
