What type of liposuction is best when doing fat transfer? I’m reading that Vaser is best, but hardly anyone does it around me.

Hello,Vaser claimed that cells were ruptured, then when fat grafting became a thing, they claimed there technology preserved the cells. Clearly, you can't trust those companies that make the machines and are trying to dupe the surgeons into buying them. The best way to harvest fat for transfer is to minimize trauma, and any energy based system like Vaser is going to heat and kill cells. 

Many varieties of liposuction are available including ultrasonic, smart lipo, laser liposuction vaser lipo, power assisted liposuction, and conventional tumescent liposuction. The results depend very little on the method used and very much on the surgeon selected. I prefer conventional tumescent liposuction with small cannulae to produce the finest result.

I appreciate your concern and confusion, however traditional liposuction is still the gold standard not only for fat removal but for fat transfer. When performed correctly, it harvests fat cells with the least amount of trauma compared to the other “miracle machines”. Your information regarding increased damage to blood vessels and nerves and increased blood loss with traditional liposuction is completely false. If anything it is less traumatic!
However at a weight of 115 pounds, I am not sure that you have enough fat for transfer to the buttocks to make a difference and I never recommend putting on weight for a procedure. Consult with a board-certified plastic surgeon in your area to explore the best options for you. Good luck.

Most practitioners who do a lot of fat transfer and grafting do not use Vaser.  There is no evidence whatsoever showing that it provides better results.  Gentle handling of the fat and careful grafting technique are far more important. rRather than shop for a technology, shop for a provider with good reviews, results and experience.

Hello, and thank you for your question. (Keep in mind that an in-person consultation is necessary for the best advice.) The gold standard for liposuction is tumescent liposuction. No other liposuction technique has been shown to be superior. Tumescent liposuction is great for fat transfer because during fat harvest (aka, liposuction) this technique does not "melt" the fat (which is what happens with Vaser and laser liposuction techniques). Because tumescent liposuction doesn't "melt" the fat, the fat cells are more likely to survive harvest and be viable during transfer. If you decide to pursue surgery it’s very important that you find a surgeon whom you are comfortable with and trust and who evaluates you in-person and tailors an individualized treatment plan for you and your aesthetic goals. Its been very hard to show that any technique for liposuction is better then the standard techniques introduced 40 years ago in terms of result. The best surgeons in the world largely eshew lasers (SmartLipo) and ultrasonic (Vaser) machines because they add cost and complications from heat without adding any clinical benefits.With fat transfer, the addition of Vaser and other technologies that produce heat is widely thought to be detrimental to fat cell viability. Most surgeons would avoid doing it on a fat transfer case and just use a low pressure suction technique with a syringe or cannula.

There are many liposuction products out there that claim to be superior to traditional liposuction, but the data just doesn't support it.  Careful technique with both the harvest and the fat preparation are the key, not the machines used.  Find an experienced board certified surgeon to discuss recommendations with you and you'll glad you did.

Due to my personal experience Vaser Liposuction is the best technique for now. I have been use it more than 2 years and so satisfied with the result. It seperates the fat from surrounding soft tissues perfectly. Also if you want fat injection at the same time vaser never changes the fat survival rate. For  less bruising, less bleeding and  better skin contraction you can choose a vaser assisted liposuction. 

you were incorrect about multiple topics. The difference between various forms of liposuction make virtually no difference in comparison to the difference among different providers. Liposuction has higher blood loss when tumescent solution is not either used or used incorrectly without giving it proper time to work.  This is the single only factor that determines blood loss during liposuction.  Injecting tumescent solution in the proper amounts and allowing at least 15 minutes before starting liposuction.  I'll too often plastic surgeons are in a hurry, do a shotty work of injecting to mess and solution and are unwilling to wait the appropriate 15 minutes before starting liposuction.  Time is money and operating room time is very expensive.  This is what creates excess of blood loss which leads to excessive bruising and everything that comes with losing blood.  Still, liposuction and rarely creates a blood loss to the point of needing a blood transfusion.  You will only find those who spent thousands of dollars purchasing a VASER system promoting how much better the use of VASER is.  Everybody touts the importance of the specific equipment they've purchased.  All the information you obtain in your quest for becoming educated is solely based on plastic surgeons self promotion.  This is why I maintain that finding the right plastic surgeon is the only thing that matters.   Veering of course by trying to figure out what the best technique is will distract you from your only needed decision which is finding the best provider.  I can't emphasize enough the importance of finding the best provider.  Be aware that real self is chock-full of people promoting themselves distorting each readers view of what's important.  The results from liposuction very more than any surgical procedure I know of. The results vary from grotesque disfigurement to beautiful body transformations. This is true regardless of board certification and even number of five star reviews.  When I see a plastic surgeon who has several hundred reviews and 80% of them are written before the patient even had the procedure I know with great certainty the reviews are highly manipulated.  Likewise, when I see plastic surgeons post after pictures with patients still having operative markings in place I know they are showing deceptive pictures since final results never look anything close to what the results look like immediately after the procedure.   Deciphering through this endless ocean of information overload to find the best provider is much more difficult than it may seem. In reality the best providers generally don't engage in social media, often don't have the most number of reviews and are busy taking care of patients instead of focusing on paid advertising and media campaigns.  Good luck in your pursuit.  Best, Mats Hagstrom MD
