Hi Latisha, Two points: 1. Buccal fat removal- I don’t think you should have buccal fat removal. Your cheek bones are already showing with a slight cheek hollow. This configuration helps to you look young, yet not look chubby. Ideal configuration, young looking and still sculpted. Buccal fat helps people look young. If you look at photos of your younger self from 10-20 years ago, you will notice that you did not have cheekbones at the time and your face may have been chubbier back then, but now it is helping you to look young. 2. Chin liposuction (AKA submental liposuction)- in general, I am not a fan of submental liposuction on people over 30 years old. To get the defined chin to neck angle (cervicomental angle) you are looking for, you will likely benefit from a Necklift with corset Platysmaplasty. How much much of a Necklift depends on your age and your skin’s ability to contract. For patients, younger than 35 with minimal skin laxity and mainly fullness, they could get away with a submentoplasty without any skin removal. This is similar to the chin liposuction, but adds additional fat removal under the platysma muscle and also tightens the platysma muscle under the chin called a Corset Platysmaplasty. The incision is about 1/2-3/4” wide versus 1/4” wide for chin liposuction. The wider incision allows for a “keyhole surgery” looking through this 1/2-3/4” incision to trim the deeper fat and clean and find the platysma muscle edges to sew them together. The deep fat removal and Corset Platysmaplasty is how we are able to consistently get sharper chin to neck angles (cervicomental angle.). The skin on younger people is elastic enough to contract like with liposuction patients. If this were a Podcast, I could keep talking for another hour, but I’ll end it here. Hello, your chin implant, as shown, is likely the wrong size and shape. Was it put in underneath your chin? It appears to be causing some fullness under the chin, and doesn't project forward enough. The right kind of implant should give you quick improvement.
