This is very helpful for patients who have some rippling around saline implants.

Fat transfer can certainly be performed on someone who already has implants, especially if the implants are placed under the muscle, but can also be performed when implants are above the muscle. Yes, a fat transfer can be done to actual breast tissue, even with implants, if there is enough breast tissue to support the fat. If you were small before implants, your own tissue has been stretched and thinned and may not support much fat. An examination will tell if you can add fat.
