Can I get a breast fat transfer while I have implants?

I had a breast augmentation with saline implants several years ago, can a fat transfer to breasts be done while these are in?

Yes, you can get a fat transfer with existing implants in place. The most common reasons for this are wanting improved cleavage or upper pole fullness (assuming your implants are in the appropriate place to begin with). If you want to be significantly bigger overall, or have more central projection of your breast, or if your implants are not in an appropriate position, you may need revision of your implants (which can be done at the same time as fat grafting).

Fat transfer can certainly be performed on someone who already has implants, especially if the implants are placed under the muscle, but can also be performed when implants are above the muscle. While one can only expect approximately one brassiere cup size increase to assure full take of fat, this procedure can embellish your breast augmentation. Yes, a fat transfer can be done to actual breast tissue, even with implants, if there is enough breast tissue to support the fat. If you were small before implants, your own tissue has been stretched and thinned and may not support much fat. An examination will tell if you can add fat.

This is called composite breast augmentation where there is implant and fat injection either at the same time or at separate times either before or after breast augmentation. This is very helpful for patients who have some rippling around saline implants.
