How can I fix my puffy nipples? Also, would I be a good candidate for fat grafting to adjust breast shape? (Photo)

Growing up, I thought I had tuberous breasts and once asked a surgeon who told me that I did not. I'm starting to think that the surgeon was incorrect and I do have tuberous breasts. I'm not sure how much fat is needed for a transfer so I don't know if I would be a good candidate. Also, how much can the shape be altered? Could my nipples point forward? Or would they simply move inward? Additionally, could anyone explain a procedure to fix my puffy nipples? I hate them the most!
