Yes, this can be improved with a revision. For best results smoothing irregularities, I recommend a hi definition procedure like VASER liposuction. I use VASER HI DEF with ThermiRF/J Plasma/Renuvion/InMode BodyTite and fat grafting to get the most fat removal, skin tightening, contour and definition. Sorry for the irregular lipo results. The only method which might improve these issues is a combination of 'off label' KyBella to lumps and than Sculptra to the multiple dents. Fees in the $10,000 ranges due to multiple sessions needed and at least 6 months of monthly visits. 

The definitive correction for liposuction deformities involves the advanced technology of ultrasound assisted VASER liposuction. As an official trainer for VASER liposuction, I encounter botched liposuction results weekly. VASER liposuction is the only modality to ensure a smooth contour regardless of severity in contour abnormalities by virtue of being able to remove any and all remaining fat comprehensively and uniformly. Dear MistyMDH,Hello and thank you for your excellent question. It is possible, but extremely difficult to resolve the issues you are identifying.  I have found that many patients can get GOOD results with liposuction alone, and EXCELLENT results with IDEAL Liposuction! IDEAL Lipo is a proprietary technique that we have been developing for the past 12 years. First off, all devices and instruments used are FDA-approved, unlike many fat harvest and transfer devices beying used today. These procedures are typically performed in the office, with only local anesthesia (although patients can request deeper sedation, if they so desire). We typically use Smart / Laser Liposuction or VASER (ultrasound) to reduce dense, fibrous fat, and stimulate collagen formation; Radiofrequency-based skin tightening (like Body Tite, or Thermi) to maximize skin contraction; and Ultrasound, RF, or subcision (Z Wave, Venus, Cellfina) to treat cellulite. Patients go home the day of surgery; typically do not use any narcotics postop for pain control, and are back to light exercise and work 48 hours after their procedures. In my practice, we specifically evaluate patients for these additional treatments, and select the appropriate treatment based on their skin quality, amount of fat, previous treatment(s), and surgical goals. It is best to answer your question during a face-to-face evaluation, when you can discuss your goals and expectations for the procedure, and you can have a physical examination to evaluate for that procedure. I recommend that you find a board-certified, or board-eligible plastic surgeon with whom you are comfortable. Ugh.  I hate to see this. SO many people are seduced by the idea of abdominal liposuction, and so many people have this outcome.Liposuction theory is the creation of tunnels through fat. The tunnels collapse and the contour flattens.  This is most effective where the overlying skin is the tightest and has the most elasticity.  Abdominal skin is not that tight, it is not stretched over any solid substructure, and it has poor elasticity.Yours is the result of poor planning and poor execution and unfortunately there is not an easy fix.  More lipo is unlikely to help signficantly, although it may reduce some contour fullness.  In my experience the skin needs to be tightened (read: tummy tuck).

It is not unusual to see irregular contouring after liposuction. There are many possibilities to try to help you. Kybella injections into the lumps is one, another is Exillis non surgical radiofrequency that helps dissolve fat. Seek the advice of a board certified physician to learn about your options and risks.
