Is this bruising or necrosis on my nipple? (Photos)

I am day 4 post op implant exchange and anchor lift...the tip of my right nipple has been darker since surgery. Today it's even darker....it still feels soft like the rest of my nipple. My surgeon said it's prob just a bruise but I am very stressed it's neceosis..

Think of the nipple-areola is part of the skin envelope of the breast, so its position does not change as implant are moved. This can create the appearance that they are to the side. Changing nipple position requires some version of a lift even if they are moved sideways and not up.

Based on your photo, the plates that you were seeing may just represent edema, or may represent some visibility of the implant pocket. Considering surgery was very recent, it will take time for things to settle into final position. I would recommend follow up with your board-certified plastic...

Your implant does seem to sit a bit higher on the right side. You may need a revision to drop this down a bit to match the crease on the left side, but it is still early days for you. It is worthwhile waiting for another couple of months or so at least, before you decide to have it revised.

Thank you for your pictures and questions. This is a common finding when doing and implant and a lift at the same time. The pressure on the areola from the underlying implant causes stretching of the areola. Placement of a permanent suture during the lift can help but it not always assured to...

It's unfortunate you aren't fully satisfied so soon after your initial surgery.  I am inferring these are saline implants on top of your muscle.  If you want to be smaller and have less rippling, downsizing to gel is a good idea and a plane switch (to under muscle) should be considered.  ...
