I am interested in Sientra round textured implants for my upcoming surgery. Can they be used with a dual-plane method? It looks as though many doctors allow this, but one or two have strong opinions against it. What does the research say?

Texturing on a round implant under the muscle may hold the implant in a higher position if the pocket is not created accurately.

When a surgeon talks about going under the muscle, they are generally referring to dual plane, which means that the upper middle portion of the implant is covered by the pectoralis muscle, while the lower and outer part of the implant is covered by breast tissue without muscle coverage.There are different types of dual plane techniques, which allow for releasing the muscle from the overlying breast tissue to allow for more expansion of the lower breast when this is needed for tuberous breasts or mild drooping. What type of implant is used in conjunction with a dual plane is somewhat irrelevant. Any type of implant whether textured or smooth, saline or silicone, anatomical or round can be used during a dual plane procedure. In general, I recommend using textured implants with a dual plane technique to maximize soft tissue coverage and shape of the breast while minimizing the risk of capsular contracture and implant migration.

Answer: Sientra implants OK for dual plane or other options depending on individual I consider 4 options for implant placement: submuscular (which is dual plane because only part of the implant is covered by the muscle), split muscle (which I do most of the time because it gives upper pole coverage without the animation problems associated with dual plane), subfascial (which adds a little bit of support but not much coverage), and subglandular (which I rarely do because subfascial is better.) Sientra implants can be used with any of these, depending on individual circumstances.

Answer: Dual plane technique for breast augmenation The dual plane technique has to do with the creation of the implant pocket, not the implant chosen. I do this procedure frequently and a textured round Sientra implant is the one I use most commonly. I think there must be some miscommunication regarding other physicians strong opinions against it.
