Thanks for your question and detailed information. Since you already have implants, I would want to know a few other items--are they submuscular and smooth? What are your goals other than increase in size and more projection? A full, high, rounder, "fake" look? More firmness? Closer cleavage? More "side boob?"Let's assume you have smooth round submuscular implants, and that you don't presently have capsular contracture, but simply want to increase size and projection. Once your present implants are removed, the already-healed capsule will not have a high likelihood of adhering to the textured surface of your new implants without first having a capsulectomy, which would be more difficult, bloody, and otherwise unnecessary.Capsulotomy will be necessary to enlarge your pockets to allow for larger implants, but complete capsulectomy to give a fresh tissue surface for textured implant adherence makes little sense to me. Especially if you do not already have capsular contracture issues. Why not simply opt for Sientra's smooth, round implants in enlarged pockets, and use the high profile (smooth round) 655cc or 695cc implants? These have base diameters of 15.1 and 15.4cm, which makes them a bit wide for your breast base even in high profile implants. Unless your breast base dimensions are measured conservatively, which I find often to be the case, you may only be able to go up by 100-150cc per side without creating new issues such as symmastia, double bubble, bottoming out, or more visible implant rippling.In other words, without consultation and more information, give your surgeon the flexibility to make recommendations based on not only the two goals of increased size and projection, but on the remainder of your anatomic considerations, present results, and careful implant selection. Smooth round implants are teardrop-shaped when standing, and are indistinguishable from textured teardrop-shaped implants in actual standing patients by mammogram. But textured teardrop shaped implants remain that shape when reclining, which is decidedly less "anatomic" and natural than smooth round implants, which flatten and round out wile sliding slightly to the side of the chest, just like natural "real" breasts. You indicated you were considering round textured implants, which would not move (because of the adherent texture), or would slide around in the already-healed capsule, negating the potential advantages of textured implants. Plus, the high-strength cohesive gel that Sientra uses means those implants would be more firm than your present implants, and less likely to assume a natural teardrop shape when standing.Consider smooth round implants in the submuscular plane, and choose the implant size and profile carefully with your chosen plastic surgeon, who should do enough breast augmentation to discuss all of the pros and cons of the various alternatives so you can make a joint decision that will best fulfill your goals.
