Hi there,Good question.  The answer is that liposuction is most unlikely to be the correct solution. If you're in the normal weight range the most likely anatomy you have is glandular hypertrophy, or true gynaecomastia. The glandular tissue is dense, and not able to be liposuctioned. The most likely best solution is for subnipple glandular hypertrophy is a careful direct excision of the dense tissue using a small incision placed at the nipple edge, and which should become inconspicuous quite quickly.Most often a little fine cannula lipo is helpful at the same time, to feather the edge of the excision into the surrounding normal tissue.  It's important that the breast capsule is disrupted during the excision as well, as it can hold a nipple base preventing it flattening if puffy, even when the gland is removed.This is a simple, day surgery procedure. Gynecomastia is a combination of breast tissue in fatty tissue. If the breast mound is substantially glandular than direct excision may be necessary. This can be determined by physical examination.

In some men, the excess is mainly fat and can be removed with liposuction. However, if you have actual breast tissue there, it may need to be excised. I would suggest meeting with a board certified plastic surgeon and discussing all your concerns.

Breast tissue is made up of both glandular and fatty tissue.  If your tissue is primarily fatty, then liposuction alone can be helpful to improve your condition.  Most people have enough glandular tissue that some direct excision is also required, in order to get the best possible results.

The term "surgeon's agent" is a little worrisome. Suggest you meet with a real plastic surgeon who does a large number of these and be properly evaluated. It's unusual to be able to treat your type of situation with out removing some portion of the gland. This can be done via a tiny incision inside the armpit with no visible scar on the chest.
