There will be some elevation of the hairline when you have an endobrow lift. The amount of lift will depend on the laxity you have in your brow. If you have a very lax brow, and you can move it much higher with your fingers, then you may get more of a hairline elevation too. Most patients do not complain of a change in the hairline as it only goes up about 4-5mm. If you have a high hairline already you should consider a pre-trichial lift where the incision is just at the hairline, as this will lower your hairline.
In my experience it depends on the amount your brow is lifted. The more it is lifted the more the hairline is elevated.. This would be more than 1-2 mm in most cases. There are other options not to lift the hairline and even lower it while lifting the brow. All choices have pros and cons. The amount of hairline elevation from endoscopic brow/forehead lift depends on exactly what is done during the operation. Your hairline with advance upward nearly the same amount that your brows are raised. If you need to raise the brows by a centimeter or two, the hairline must also move upward the same amount. If you are concerned about moving the hairline, a pretrichial forehead lift would be better. It keeps the hairline in the same location, while raising the brows. And most surgeons will admit that it is longer lasting than an endo-brow because redundant skin is actually removed, not just repositioned.
If you have a high forehead then you would want to perform the operation in such a manner to not make the forehead any higher. There are many techniques to perform a brow lift and your plastic surgeon should be able to help you select the method that is best for your anatomy and expectations.
You may wish to consider a trichophytic hairline incision to avoid elevating the hairline more than a few millimeters, as this approach will not raise the hairline. It may even lower the hairline a bit if that is one of your objectives.
