23 years old. Would a mid facelift a better option for me? (Photo)

I am sorry that you have had such
disappointing results from fillers, but I am not surprised. They are temporary
and can give only modest changes in terms of volume. A midface lift is not the
right approach because at age 23 you do not have any real skin or deep tissue
laxity, which is what that operation will address, and therefore a midface lift
would give very subtle results at best. Cheek implants can easily be overdone,
but a small implant will add volume to your cheeks in a meaningful way. I would
wait until the fillers have resolved (with RADIESSE or Perlane, it will take a
year), and then consider an implant. Fat grafting is another alternative that
can very successfully augment the cheek and may have a more pleasing and
longer-lasting result than fillers. Good luck with your decision.

Based on your pictures a mid face lift would not give you the results you are looking for. You may consider a cheek implant along with fat grafting. 

I definitely would not recommend a facelift. Conservative use of fillers the nasolabial area and possibly the cheeks is a consideration. The skin appears to be in excellent condition.

If you need more cheek structure, especially at a young age, an implant is a better choice than fillers, fat, or a mid face lift. Filler isn't always stiff enough to suspend a mid face. Also, by cal fat extraction could help to rebalance your facial volumes. 

One or 2 sessions of fat grafting would be better since the fat can take as a graft and give you a more permanent correction.  I agree volume is the answer and not skin excision via a face lift. 

Surgery at too young an age can result in an unnatural appearance because you are trying to reverse changes that haven't happened yet.  I would suggest continuing with the fillers.

Given the amount of fillers you have had, I would recommend you allow time for these to resorb before you consider more permanent options.  While implants are an option, other considerations would be fat grafting or a different filler such as Voluma which is more effective when volume and support are lacking.  A mid face lift elevates a photic midface, but likely would not address your genetic midface hypo plasia. In my opinion you are a perfect candidate fore the mid face "Silhouette Lift" + HA fillers to the nasolabial/marrinontte lines. Or fat grafts to them... Fee in $5,000 range. 

A few comments:1.If you require so many syringes at your young age, rest assured that number will rapidly increase as you get older.2. Your supportive tissues are youthful and are most likely maintaining the malar fat pad position. Therefore, in my opinion, it is  unlikely you will get significant elevation but will risk permanent change to your lower eyelid shape.3. Your issue is (there are no pretreatment photos)probably as you stated, malar hypoplasia. Therefore, the "cure" is volume not descent. Fat grafting is frequently employed in such cases, providing all the volume you require, will slow further bone aging and saves a tremendous amount of money relative to fillers----as long as it is done right.Hope this helps
