I did an upper blepharoplasty slightly more than one month ago (Surgery dated 19/5/2015) keloid like scars seems to form at the inner eyelid. I've consulted 3 doctors so far where 2 of them concluded that I can only smoothen the keloid scarring through laser. It has slightly flattened and less reddish after I went for a steroid injection (Triamcinolone) but the itching is back. Please help!

This is a medicine that is gaining popularity for wound modulation. Historically it has been used for glaucoma surgeries and is a chemotherapy agent. My advice is to encourage you to understand that this will take time and a few additional treatments to the scar itself. Your surgeon may use a combination of kenalog, 5FU, and laser. All these have been shown to improve the type of scar thickening that you have. Be patient, and you will enjoy a great result. I wish you the best.

Eyelid incisions usually heal very well, although some require steroid or 5FU injections to calm down the scar formation if you are prone to keloids. Lasers help too - although rarely are needed. I would continue to follow up regularly with your surgeon and be patient. Until at least 3 months after surgery, you cannot tell what the final scar result will be. Good luck.

Answer: Hypertrophic scarring of the epicanthal fold This should be monitored very closely as you want to avoid webbing that can be caused by scarring in the epicanthal fold. The most common way to prevent any issues is serial injections of 5fluorouracil (50mg/cc) directly into the lesion every 1-2 weeks. Once abated you can think about using a laser to remove redness of a hypertrophic scar but it's too early to evaluate the need for that. If the 5FU doesn't show improvement you can always mix in a little triamcinolone with it but better to avoid the steroid unless needed to avoid atrophy surrounding the scar
