You have some inflammation from the chalazion surgery. This usually resolves within a couple of weeks. While healing it may contract and that is why the eyelid is pulling upward. Would benefit from an injection of steroid to decrease the scarring and soften the lid.
Healing is a continuum, and swelling continues to settle over several weeks. If the procedure was recent, your concerns may improve with time. If your surgery was recent the residual inflammation will resolve with massage and/ or kenalog. The upper eyelid contour will improve as well. This is assuming the chalazion was completing removed. You should follow up with your surgeon.
You have some residual inflammatory changes with some thickening either from the procedure or the original chalazion. This change in thickness and contour is elevating the height of you crease fold in that area. If the excision was recent (less than 6 weeks) this will usually go away on its own if the chalazion was debrided thoroughly during the procedure.
From the photo, it appears that there is some scar tissue on the right upper lid presumably from the chalazion removal. Any spontaneous improvement depends on how long it has been since the chalazion was removed and how much soft tissue had to be removed around the chalazion. , If it was recent, the lid position may improve with gentle massage and/or a Kenalog injection to reduce the contraction of the skin over the lid which pulls the lashes up. If has healed completely, and the scar is mature, a relative ptosis repair on the left upper lid to lift the lid above the pupil to a similar level would improve the symmetry. Once the amount of space above the pupils are the same, consideration for removing a pinch of skin from the eyelid above the crease can open the eyes even more. John Standefer MD
