At 5 years after liposuction, massage is unlikely to make a meaningful change if the lumpiness has been stable. Massage can sometimes help early postoperative swelling or firmness, but mature contour irregularities usually come from uneven residual fat, scar tissue, adhesions, skin laxity, or a combination of these. The right treatment depends on what is causing the irregularity. If there are small areas of residual fat, careful revision liposuction may help. If there are dents or tethered areas, fat grafting, subcision, or scar-release techniques may be more appropriate. If the skin itself is loose or crepey, skin tightening or a skin-removal procedure may be needed, and liposuction alone could make the unevenness worse. An in-person exam with a board-certified plastic surgeon is the best next step. They can examine the abdomen standing and lying down, pinch the skin and fat, and decide whether revision liposuction, fat grafting, energy-based tightening, or no further surgery is the safest option. Until then, gentle massage is not harmful if the skin is healthy, but I would not expect it to correct a 5-year contour problem by itself.
