The photo shows a small area of fullness or a soft tissue fold just behind and below the ear, near the upper neck/lower scalp. Liposuction may be possible if this is truly a small pocket of subcutaneous fat, but this area is more delicate than typical neck or body liposuction. The tissue can be fibrous, the hair-bearing scalp is nearby, and over-resection can create contour irregularity or a visible indentation. An in-person exam is needed before choosing the right treatment. A surgeon would need to feel whether this is fat, thick skin, a fold, scar-like tissue, a lipoma, or another type of lump. If it is a discrete lipoma, direct excision may be more appropriate than liposuction. If it is loose skin or a natural neck fold, liposuction alone may not help much. Cost varies widely by surgeon, anesthesia, facility, and whether it is done alone or with another procedure, so a consultation is the only accurate way to quote it. I would see a board-certified plastic surgeon or facial plastic surgeon who does neck contouring and ask specifically about the risk of a visible contour change in that location.
