Sliding genioplasty or chin implant - Which procedure would deliver a more balanced look? (photos)

An implant would be hard to make look really good. Your chin is short as you noted, and increasing face height significantly in a chin like yours is not predictable with an implant. Genioplasty would allow for the additional height and projection to balance out your profile as well as facial 1/3s. Your lower lip would be much better supported and in a better relation to your upper lip so your upper lip support will probably get better too. That said, you look to be 18+mm in your upper lip skin so have some room to go up with additional lip lift at the same time as well.

All things considered, you may be a candidate for either procedure. In females, the larger implants can look wide and masculine. So if you need 4-6 mm advancement, an implant is fine. When 7-8 mm of forward advancement are indicated, I prefer the genioplasty procedure. Occasionally a patient will have a personal preference of one over the other and that is fine as well.

It would be very difficult to give you a sound advise based on these photos. In person consultation with a board certified plastic surgeon with good reputation in facial implants and facial rejuvenation, is recommended. Good luck.

A sliding genioplasty is performed by an oral surgeon in a hospital setting under general anesthesia with an overnight stay at the hospital and is much more invasive. Placement of a chin implant can be performed under local anesthesia in thenoffice setting as an outpatient procedure which takes about 30 minutes.
