Best procedure to improve my facial profile (chin implant, genioplasty or orthognathic)? (Photos)

An implant would be able to get you close to your 10mm projection in profile, but you start with a wider chin (width outside commissure) so doing an implant would probably look too wide from the frontal view. Genioplasty you could get 10-12mm…your 15mm projection is a bit too much (chin beyond lower lip) and 12mm is about as far as you can predictably go. This would be the best way to get good projection, keep your chin shape (no widening) and give you the best labiomental aesthetics. I almost always prefer genioplasty in younger patients as well, that way you aren’t worried about what will the implant be doing in 40 years.
Finally, jaw surgery is an option if you want to fix the mid face as well…you have an obtuse nasolabial angle so if you want a great airway, optimized profile, and don’t mind having a bigger procedure, jaw surgery would be best. I would say if you snore or your doc looks at the ceph film from your ortho and you have a narrow airway, give a double jaw serious thought.

If your bite is functionally fine then you don't need orthognathic surgery. For small advancements a chin implant works well. In cases that I advance greater than 7 mm I will recommend a sliding genioplasty. Both procedures are performed in the office with sedation anesthesia. The results are similar.
