Hello! Great questions. There are different ways to approach rhinoplasty. The two major are the closed versus open approach. The latter in my experience affords for greater precision and changes. To increase the length and perform changes at the tip, the natural cartilages would be repositioned and grafting material in the form of cartilage used to provide structure. By doing so, you can create lasting, natural changes to your nose. This is called structure rhinoplasty and it works very well. You can certainly create nice changes to your nose if done well.
